The Center To Rise Wellness Spa Studio (CTRWSS) has an array of eclectic staff: Licensed Clinian(s), Empowerment Specialist, Energy Healers and volunteers.
Christina Butler, LPC is a Licensed Professional Counselor licensed by the Tennessee Department of Health Division Of Health Related Boards. I am currently awaiting reciprocity with the State of Texas State Board of Examiners of Professional Counselors. Christina is the clinical director of the Center To Rise Wellness Spa Studio (CTRWSS) Texas and Tennessee Division.
Nicole Butler, LMSW is the Chief Executive Officer at Center To Rise Wellness Spa Studio. Nicole will provide non-clinical services to clients. Per Texas State Board of Social Worker Examiners About the Profession – Scope of Practice. §781.202. The Practice of Social Work section (e) states the following: (e) Independent Practice for LMSWs–An LMSW recognized for independent practice may provide any non-clinical social work services in either an employment or an independent practice setting.
Confidentiality I will treat with great care all information you share with me. It is your right that our sessions and my records about you be kept private. In all but a few rare situations, your confidential it is protected by state law, the rules of my profession, and my personal integrity. Texas and Tennessee state law requires me to inform you that in certain cases your confidentiality is not protected, and your information may be disclosed to the appropriate authorities/agencies. These cases are:
- • If a court of law issues a legitimate subpoena for information stated on the subpoena.
- • If a judge signs a validly executed search warrant or compels clinician(s) to testify in court.
- • If a client is in therapy or being treated by order of a court of law, or if the information is obtained for the purpose of rendering an expert’s report to an attorney.
- • If a client threatens or attempts to commit suicide or otherwise conducts themselves in a manner in which there is a substantial risk of incurring serious bodily harm.
- • If the therapist has a reasonable suspicion that a client or other named victim is the perpetrator, observer of, or actual victim of physical, emotional, or sexual abuse, or suspected neglect of, children under the age of 18 years, persons with disabilities, or elderly persons.
- • Information may be provided to parents if the client is a minor.
- • If you file a complaint or lawsuit against the clinician, I may disclose relevant information to defend myself.
- • If you die and the communication is important in matters affecting your estate.
- • If I find that you are seeking treatment to avoid detection or apprehension, or to enable anyone else to commit a crime.
- • If you disclose a sexual relationship with a past mental health professional or serious misconduct on the part of another licensed professional, I am obligated to report this to the proper authorities and licensing entities.
- • Should you want clinician to acknowledge you as a client or to disclose information to any person, and have verbally or in writing requested clinician to do so.
- • If at any time, I have concerns about a particular issue, Center to Rise Wellness Spa Studio (CTRWSS) clinician(s), may discuss that issue with a qualified and trustworthy attorney, but the privilege extends to that attorney who must also maintain this confidentiality and may only discuss the issue with a clinician(s)
Additionally, the clinician may disclose the information if you sign a release form granting permission to designated third parties to receive information that you request CTRWSS to share. I will never disclose your information for any reason without your knowledge of the intent.
Therapeutic Relationship The relationship between CTRWSS Staff and clients is the container through which change can take place. As such, the relationship is often one in which close emotional bonds develop. It is also a professional relationship, in which appropriate boundaries must be maintained. The therapist-client relationship is so important, I cannot be involved in a social relationship or friendship that exists outside of the therapy room. Limiting our relationship to the therapy office keeps your therapeutic environment safe, secure, and free of outside complications that could interfere with your therapy work. The Therapeutic Process entails tailoring therapy to meet the needs of each client individually. I have worked with clients experiencing grief, financial difficulties, relationship difficulties, and those who are alone without family support.
Fees: $55 per 50-minute session. $35.00 per 30-minute session. $125.00 per 1.5-2 hr Energy Healing session. Deposits are 50% of the session of choice. I.e if the session is $55.00 the deposit is $27.50. Full Payments and deposits can be made on http://www.Centertorise.com
Session Guidelines sessions are 30, 50,minutes and 1.5- 2 hrs. If you need to cancel an appointment, you must give 24 hours notice. Otherwise, you will be charged for the missed appointment. Sessions are expected to begin and end at the scheduled time. Late arrival on your part will not extend the scheduled ending time for a session. It is expected to be on time, and Center To Rise Wellness Spa Studio will make appropriate remedies if the clinician/practitioner or empowerment specialist is late, such as by making up the time, prorating the fee, etc. The frequency of sessions and the length of the empowerment sessions are aspects of the work that you choose to do.
LENGTH OF TREATMENT Some clients need only a few empowerment sessions to achieve insight toward their goals; others may require months or even years of counseling. As the client, you are in complete control and may end/terminate or cancel the CTRWSS counseling relationship at any time. When you are ready to leave therapy, all you have to do is give CTRWSS advance notice. I may ask that you participate in a termination session for the purpose of reviewing our work together and creating a positive ending.
INSURANCE CTRWSS does NOT accept insurance. We are a private pay entity.
COUNSELING APPROACH CTRWSS philosophy can best be described as relationship-based and eclectic in nature. Rather than adhering to any fixed approach, Speak & Release should be as unique as the individual, and that engaging a warm, genuine relationship is the essential factor in helping clients work towards their desired outcomes. We believe in the importance of the whole person and in collaborating to maximize the strengths, resources, and resilience that clients already possess. We encourage individuals to explore meaning, become aware of our own possibilities and limitations, navigate the complexities and uncertainties of life, and learn to live authentically.

Coaching Informed Consent

Life Coaching Agreement
1. Life coaching is not currently a regulated industry in the state of Texas. It will be solely the responsibility of the client to determine the effectiveness of the services rendered and the competency of the coach.
2. Life coaching is not psychotherapy or counseling. Life coaching does not address mental disorders as defined by the American Psychiatric Association. Your life coaching sessions are not a substitute for counseling, psychotherapy, mental health care, or substance abuse treatment. Psychotherapy is a healthcare service and its primary focus is to identify, diagnose, and treat nervous and mental disorders.
3. Life coaching assumes the mental health of the client. Life coaching is a collaborative process that is present and future-oriented. It is action-oriented, solution-focused and encourages change. It involves accountability and commitment to growth through increased competence, commitment, and confidence.
4. As the client, you set the agenda for these sessions and your success will largely depend upon your willingness to define goals and try new approaches.
You can expect Empowerment Specialist, Energy Healer, as your life coach, to be honest, and direct, asking straightforward questions and offering challenging techniques to help you keep moving forward. Confidentiality Confidentiality is an important element of the coaching process. Your identity and ongoing work will be kept strictly confidential. I will only release information about our work with your written permission, or if I am required by court order.
The following exceptions will apply:
1. There are a broad range of events that are reportable under child protection statutes. Physical or sexual abuse of a child will be reported to Child Protective Services. When the victim of child abuse is over age 18, I am not legally mandated to report it unless I believe that there are minors still living with the abuser who may be in danger of being abused. Elder abuse is also required to be reported to the appropriate authorities.
2. If you are at imminent risk to yourself or someone else or make threats of imminent violence against another person, I will take appropriate action.
Fees, Payment Schedule, and Services 1. You agree that our life coaching session will take place, in whichever manner you prefer (via in person, by phone, or the internet). Each appointment will last for 30/50 minutes and the fee is $35/$55. Energy Healing Sessions are 1.5-2 hrs at $125.00. The payment is due at the start of each session.
Note: that there is no insurance coverage for these sessions.
2. I request that you provide at least a 24-hour notice if you need to cancel or reschedule your appointment, otherwise you will be charged for the session in full. Acknowledgment and Release of Liability By signing this disclosure and consent statement, I acknowledge that I understand the above information. I agree to hold harmless Center To Rise Wellness Spa Studio and my life coach from all liabilities and claims which may arise as a result of my participation in life coaching.
